The Family Reunification Model (c) is a logic model designed specifically to facilitate efficient stepdown for high-risk youth, ages 6-18, with mental, emotional, and behavioral issues, whom are placed outside in the home; specifically in a mental health Residential Treatment Center. The FRM was created from years of direct service & deliberate practice process evaluation with children, teens, and families with multi-systemic involvement in an Ohio Residential Treatment Center. The Family Reunification Model is a platform for Residential Treatment Centers to use to coordinate care and treatment to achieve the best outcomes. The overall goal is to improve outcomes for successful reunification or re-integration to a less restrictive setting.
Objective: The FRM aims to reduce LOS, identify and advocate for clinically indicated LOS (with the use of the Prognosis Factors(c) Theory and predictive analytics), and enhance the collaborative process of service delivery.
